Yes, a suppressed immune system can affect dental health and teeth in several ways. Since the immune system plays a crucial role in fighting infections and maintaining overall health, its suppression—whether due to an underlying condition, medications (such as corticosteroids or immunosuppressants), or autoimmune diseases like juvenile myositis—can lead to the following dental concerns:

1. Increased Risk of Gum Disease (Periodontitis & Gingivitis)

  • A weakened immune system makes it harder to fight off bacteria in the mouth, increasing the risk of gum infections, inflammation, and gum disease.
  • Symptoms may include swollen, bleeding, or receding gums.

2. Higher Susceptibility to Cavities

  • Reduced saliva production (common with some immunosuppressive medications) can lead to dry mouth, which increases the risk of tooth decay and cavities.
  • Bacteria thrive when the immune system isn’t strong enough to control their growth.

3. Delayed Healing After Dental Procedures

  • A suppressed immune system slows down wound healing, making recovery from dental work (such as extractions, fillings, or oral surgeries) take longer.
  • This can increase the risk of post-procedure infections.

4. Increased Risk of Oral Infections (Thrush & Ulcers)

  • People with weakened immune systems are more likely to develop oral thrush (fungal infections caused by Candida).
  • Mouth ulcers (sores) may occur more frequently and take longer to heal.

5. Bone Loss & Tooth Weakness

  • Long-term steroid use (common in autoimmune conditions like juvenile myositis) can lead to bone loss, affecting the jawbone and potentially leading to tooth loss or dental instability.

How to Protect Dental Health with a Suppressed Immune System

  • Maintain good oral hygiene: Brush twice daily, floss, and use antimicrobial mouthwash.
  • Regular dental check-ups: More frequent visits (every 3–6 months) may be needed.
  • Stay hydrated to combat dry mouth.
  • Avoid sugary foods and drinks to reduce cavity risk.
  • Talk to your dentist about fluoride treatments or prescription mouth rinses for extra protection.

Tips from a Parent:

  • See a dentist at your children’s hospital if possible. Sharing records and consulting with your rheumatologist can be helpful in coordinating care.
  • Schedule cleanings every 8–12 weeks to stay ahead of potential issues. These typically cost around $100 out of pocket and are well worth it. (Your rheumatologist can submit an appeal to help get dental insurance coverage. Ask them to send the letter to you so you can submit it each time insurance denies coverage.)
  • Prioritize extra oral care—regular brushing, flossing, and using mouthwash can help prevent complications.
  • Inform your dentist about periods of immunosuppression. A proper immune response is needed for baby teeth to fall out naturally. If this process is suppressed, as it is for most of our children during these baby tooth years, the baby teeth may not loosen on their own and might require extraction.
  • Keep your dentist informed of all changes to your medication. You may ask your dentist if they would be willing to connect with your rheumatologist by email so they can consult on your child’s dental care.
  • Mouth sores. If your child is experiencing mouth sores, ask your rheumatologist and pharmacist about “magic mouthwash” which can help.
